Before your appointment

What to bring

  • Insurance card (many insurance providers require you to have a referral or authorization from your primary care physician before treatment by a specialist; check with your insurance provider if you have any questions about your plan or coverage)
  • Government-issued photo ID
  • Any splint, brace, or other medical devices you may be using for your injury
  • List of all medications (over-the-counter, vitamin/herb, and prescription) you take, including doses and how often you take them
  • X-rays, MRIs, and any other imaging (not older than six months) related to your injury or condition
  • Clinic notes or operative reports from your referring physician related to your injury or condition
